do I need some kind of permission from rental or documents for a bike to go with him to Phangan? Do they ask something at Raja Ferry when boarding? We have a bike for rent, no documents, no lease agreement either, with a red number for some reason.

They don't ask anything, but in any case you need to coordinate the departure with the owner of the bike. Usually, if the departure is allowed, then in case of a breakdown and an accident, the return to Koh Samui is at your expense.
