1 Copy of passport front page 2 Copy of passport page with valid visa/stamp 3 One of: Copy of Work Permit with extensions and registered address Resident Certificate (obtained from Immigration Office) Copy of Yellow Book (for real estate owners) 4 Medical certificate from clinic, hospital. Note that if you are applying for two driver's licenses category A and B, you will need two sets of documents, because you will actually receive two driver's license cards.
