Hello, and where to look for a list of forms to fill out on a visa extension by covid stamp? And what is a certificate from our consulate?

Documents required for Covid-19 stamp/visa (60 days): Passport Copies of pages: with photo, visa, last stamp received at the port of arrival, immigration card (TM6), last stamp from Immigration Thailand (on each copy must be signed) Color photo 4x6 1900 Baht Completed forms: TM7, STM2, Affidavit Covid-19, TM30 P. S. on TM7 next to the photo you must indicate the contact phone number
