what documents are needed for arrival in Phuket, in printed form? And do I need to notarize them?

You don't need to certify anything. Upon arrival, you need a passport, a PCR payment check, a PCR test that before departure, a taipas, a hotel reservation. They can also ask for a return ticket.

A notary is not needed, it is better to print out everything that is.
