I sent the documents to the embassy, I was sent a form to fill out to carry the animal. Tell me, apparently it needs to be filled out by hand? And then send to the Thai Embassy? Or how? Or just have it with you?

Fill out the application form r1-1 (sent from the veterinary service). All this must be sent (maximum 60 and at least 14 days before the planned date of arrival) to the e-mail address of the airport where you will fly (Phuket or Bangkok). In response, they will send you an Import Permit. It must be printed out and have it with you when registering the animal at the airport of departure. The validity of the permit is 60 days from the date of issue.
