If you have a transit flight further to Phuket, without picking up your luggage, then passing through customs in Phuket is most likely.

If there is only one ticket, then the luggage should not be picked up. But you will have to go through passport control.

Yes as well as in Phuket, only in Bangkok. Passport control, and then on to the flight to Phuket.
