We did a visa there in early February. Paid hotel reservations ( booking with cancellation : 2 weeks + another week another hotel took ) Ticket to Thailand - we had a ferry from Langkawi to Koh Lipa, a ticket from Tai ( the reservation was unpaid), an extract from the card to the English and the movement on the bill for 3-6 months ( on the Russian was) A copy of the main page of the passport and a copy of the page with the stamp of entry into Malaysia. Questionnaire, checklist give on the spot everything
