If you are now on a Phangan, then only a ferry to the mainland and a taxi to Bangkok or Surat Airport.

Ferry, bus. Drives on my 9 hours. Take another look from Surat Thani airport. To it ferry 2.5 hours and 1.5 hours on mine but check.
