It is impossible to register on Koh Samui, the nearest one is in Phuket. Fill out a sample from the Phuket. mid. ru website, send it to them by email and they will set a signing date.

On Koh Samui, there is no bcc Embassy or consulate in Phuket (but I'm not sure about the message)
