Where is it more profitable to make medical insurance with minimal coverage? Is it possible to apply for 1 month, and then, while already in Thailand, extend it?

I took some kind of Renaissance for a month, came out in pennies, but it's still unreliable, it's unknown how the Russian insurance companies will behave with the local insurance, and those with clinics.. In Thailand, I plan to make local insurance. In principle, if you have already flown out of the country, then you will not be insured normally.
