We live in Pattaya. On Monday by appointment we go to the consulate in bangkok for affidavit to get the card is it better to go straight to make a card in bangkok bank in the city of bangkok? Will they issue the card right away ?

In Bkk you can avoid insurance, in Ptt it is unlikely.

They will give you a card right away in bkk. It will probably come out cheaper than in Pattaya. But keep in mind that if the card is bkkokskiy, when you withdraw cash in Pattaya, there will be a charge of 20 baht.
