I'm getting a new passport, or rather I want to get it. As a child, I changed my last name - after that I received several foreign loans for a new last name. Now I can't find the certificate of the name change surname. Is it very critical? Do I have to specify?

If you already have a new passport for a new surname, then you don't need to. You need the original certificate only if you want to change it . And so you will come the current surname first, and then the previous ones separated by commas.

I changed the child's last name in 6 months - I did not indicate it at any passport submission) there were never any questions.
