ATM ttb bank ate the card, has anyone encountered how to return?

50 to 50, sometimes return, but there are ATMs where the card is simply cut. Take a picture of the ATM with its number, address and location and with all the docks stomp to the bank branch and apply for a refund, but again, the bank did not give the card to my friend, but gave it to me since I left it in a good place, at the airport and made me take it to him and there was just the same at the ATM there was an exchanger and Madame opened the ATM and gave the card after filling out the return questionnaires.
