Has anyone ordered appliances from Amazon to Phuket? What is their fee? Are there a lot of problems with customs clearance?

Yes. Get ready for a ~30% duty. But it can slip through. There are warehouse services that will repack your MacBook in a regular box and send it as used for less money, but also without a guarantee that a sneaky customs officer will get caught and will not stick +30%.
