Vet passport, vaccinations, chip (!), medical certificate (five days before departure and only in state veterinary clinics). Send on the list everything to the vet service of Thailand (it is easy, passport photo number chip photo animal data and flight data) get permission to import. At the airport, exchange the certificate of the veterinary clinic for an interlunar one. Upon arrival, go to the veterinary service to submit documents. Personal experience, we're here with the dog right now.
