I wanted to know if I fly from Phuket to Singapore, I need to take a COVID test now and is it possible to go to the city for 2 days on a transit visa?

PCR is needed. I passed today, there is a Clinic on Patong. 48 hours before departure is required. 2790 baht - in the morning you will hand over, in the evening it will be ready. For 2 days you can get off in transit, if you have a ticket to the 3rd country (not back, to Thailand)
