I Tai passes were given simple with confirmation from Booking in Phuket, without names and surnames.

We wrote to the hotel itself (you can contact through their website or through booking) with a request to send confirmation of the paid reservation with the names of all guests. Write a request, send guests' passports, dates and reservation number.
