Documents required for Covid-19 stamp/visa (60 days): Passport Copies of pages: with photo, visa, last stamp received at the port of arrival, immigration card (TM6), last stamp from Immigration Thailand (on each copy must be signed) Color photo 4x6 1900 Baht Completed forms: TM7, STM2, Affidavit Covid-19 P. S. on TM7 next to the photo you must specify the contact phone number
