Please tell me where in Phuket they put the vaccine Pfizer or Moderna?

Nowhere. There are none at all in Thailand. Pfizer seems to have been promised to do it later this year (at a crazy price) in the spring. But apparently it was just an assessment of the market. Apparently there weren't many people.

Thank you all for the advice. My conclusion is, you need to go to Phuket BBC hospital and make an appointment there for vaccination.
